Bradley Medication Pick up by June 3rd!

Medication Pick Up Letter – English

 

It is that time of year again! All medications must go!

If your child currently has any medications and/or medical equipment in the school
health office, please pick them up on Thursday, June 2nd or Friday, June 3rd. We will
have a table outside the school’s front doors at pick up on Friday, June 3rd from
12:15-12:30. If you are unable to pick up medication on one of these days, please
contact the front office or health office, and we can arrange for another pick up. Our
goal is to not have any medications left in the health office, as they will have to be
destroyed if they are not signed out by a parent. We cannot send medications home
with students.
If your child is participating in a summer program: Medication will NOT automatically be
transferred to the summer school program. All medication must be picked up by
parent/guardian and given to the site on the first day of the summer program.
*IF ARRANGEMENTS HAVE NOT BEEN MADE FOR MEDICATION PICK-UP, ALL
REMAINING MEDICATIONS WILL BE DISPOSED OF

This form must be completed and signed by a medical provider and signed by
parent/guardian. Just a reminder that all prescription medications must be in the original
pharmacy labeled container and the label must match the medical provider form. All
over-the-counter medications must be in the original package, unopened and labeled with the
student’s name, and also match the medical provider form.
